First-Time Fillers: A Patient’s Perspective

I was 31 years old when I first had Botox injections. Up until that point, I had really only done peels and facials. I’ve never liked needles, so the thought of them going into my face was not one I was fond of. However, I also was not fond of the creases between my eyebrows, so off to the dermatologist I went. And when I saw the results, I wished I had started sooner.

When deciding where to go, I did my research. The one place I kept hearing wonderful things about was Art of Skin MD – that the facility is state of the art, the providers are the best of the best and the staff is wonderful. It made my decision easy, and I scheduled my consultation. From the moment I made my initial appointment, they lived up to their stellar reputation. And upon arriving to my appointment, everything I’d heard about Art of Skin MD was absolutely true. Another one of the top reasons that I came to Art of Skin MD as a patient is that looking natural is incredibly important to me, and they are known for doing just that. It’s not that I don’t want to age, it is that I want to age gracefully.  I immediately loved that both Dr. Palm and her Registered Nurse, Jessica Scott, approach toxins with that in mind – my face looks fresh, never frozen.

As time went by, I started to notice some fine lines around my lips from all of the years of using straws to protect my teeth from my love of coffee, tea and red wine. This, coupled with the volume I had lost in my cheeks causing marionette lines, made it clear that it was time to think about fillers.

In the conversation I had with Jessica, she answered all of my questions and eased all of my fears regarding looking “done” or “fake” – and I knew I was in great hands. She talked to me about medications and foods to avoid leading up to the injections to prevent bruising, and I started taking Arnika Forte to help further prevent bruising and swelling. We decided on a Restylane Lyft Package – this included Restylane Lyft and Restylane – the two fillers that would be best for what I needed. We started the process with before photos, which is exciting because it’s easy to get used to what I see in the mirror daily and forget what I looked like even a few weeks ago compared to now. Then, she applied topical numbing cream on my face like a mask. I sat comfortably and worked while my face numbed for an hour. Then, she cleaned the numbing cream off of my face and it was time to get started.

She talked me through every step, “you’re going to feel a little poke here” and “one, two, three” then the tiny poke (which I barely felt). She created volume in my cheeks, filled my lip lines and put a little Restylane in my nasal labial folds to smooth out the lines that had been created from that volume loss in my cheeks previously. She then handed me an ice ball, and told me to ice for 20 minutes on and 20 minutes off that day. I instantly loved the results! Friends who have seen me since have commented on how rested and fresh I look, which leads to me telling them about my experience of doing fillers with Jessica!

When having fillers done, the injector is truly an artist and the patient’s face is the work of art – but – it’s also your face. Keeping that in mind, it is imperative to trust that the injector is both incredibly knowledgeable and skilled to ensure that the outcome will be you looking like a well rested, fresher version of you. For me, Jessica and Dr. Palm both embody everything a great injector should be and I couldn’t be happier with my results!

Restylane Refyne & Defyne – The New Fillers on the Block

           Art of Skin MD is one of the first practices in the San Diego area offering Galderma’s newest members of the Restylane family that are changing the game with a very exciting new ability – flexibility. What makes this so exciting? Flexibility means a more natural look, and in the hands of the right injector, that can be game changing.

            Dr. Palm serves as an advisory board member, clinical investigator, national speaker and physician trainer for Galderma, meaning that she has been selected to educate other physicians on products’ use and safety. She has also assisted in studies related to Galderma products. Both Restylane Refyne and Restylane Defyne were FDA approved in December 2016 and released in the U.S. in January 2017. Prior to this, Dr. Palm was among the first U.S. physicians to inject the product and was selected to learn from Canadian experts outside of the country on injection techniques prior to Restylane Refyne and Defyne becoming available in the U.S.

          What makes these fillers different from other hyaluronic acid fillers already available? The hyaluronic acid strands of both Restylane Refyne and Restylane Defyne are bonded together in a way that creates a smooth gel that may provide not only a longer duration and less swelling than some other fillers used for wrinkle reduction, but also allows for more movement due to its flexibility. One of the most dreaded fears of patients is looking as though they have an unnatural appearance, and this is when choosing the right injector can make a huge difference. Dr. Palm is careful to select the right injectable dermal filler for each patient, and with the smoothness of both Restylane Refyne and Defyne, when injected by expert hands, they lend themselves to a very natural result by integrating into the areas of injection extremely well.

Total Facial Rejuvenation with Sculptra and Ultherapy

Sculptra: Rebuild Lost Collagen, Lift Tissues, Fill the Face!

Sculptra is something I have thought a lot about lately.  Right now I am in the process of writing a book chapter on this facial revolumizing agent.  Pouring over the literature I feel like I am bursting at the seams to share some of the insights about our use of Sculptra over the past decade.   Inspired by this knowledge, here are three short video vignettes highlighting the use of Sculptra in the upper face (temples and brows), midface (cheeks and lateral jawline), and lower face (chin, nasolabial folds, and marionette lines).   Sculptra to the Upper face:   Sculptra to the Mid-face:   Sculptra to the Lower face:     The new face of rejuvenation is natural, subtle, and truly restoring.  We now understand the important role of fat compartments and bony loss in the aging face.  Thankfully, we can disguise many of these changes through volumetric restoration of the face with today's fillers.   Sculptra has the distinct advantage of gradual, long-lasting volume replacement.  It works by stimulating our own body to produce collagen in the areas of placement, a process called "neocollagenesis."  Therefore Sculptra is termed a "biostimulatory agent."   Sculptra can be combined with other procedures for a synergistic effect.  While Sculptra provides "lift" to tissues through volume re-inflation, combining this injectable with Ulthera, an ultrasound technology can provide superior results in terms of lifting the brow, defining the jawline, and improving jowls.   Other fillers can be combined with Sculptra use.  The temporary numbing effect for Sculptra injection makes it an excellent complement to use of hyaluronic acid fillers such as Belotero, Juvederm and Restylane around the mouth and lips.   Voluma, a new hyaluronic acid filler from the Juvederm family of products, acts in a similar fashion to the deep filling effect of Sculptra.  It could be used initially to help fill the face as Sculptra is used over time to gradually rebuild other areas of collagen growth over time.   Interested in more about Sculptra, Voluma, or other fillers used in facial rejuvenation?  Call us at Art of Skin MD at (858) 792-7546 or contact us through the Art of Skin MD contact page.

Chest Rejuvenation? Sculptra is Here to Help

Our face isn't the only skin we need to take close and careful care for as we spring into summer.  Most of us are more cognizant about protecting our facial skin, but we neglect to take care of our necks, chest, and hands.   Over time, exposure to sun and environmental elements can cause telltale aging changes to the chest.  UV exposure damages collagen in the second layer of our skin called the dermis.  This loss of support results in skin textural changes and vertical wrinkles in the decollete area.   So what can be done to improve the appearance of this aged skin?  Sculptra!  Completed as a series of treatments, Sculptra injections to the chest promote new collagen growth, strengthening and thickening the soft tissue and skin of the chest area.  This increased support and scaffolding lessens the appearance of skin irregularities and lines in the V-shaped area of the chest.   Watch the YouTube video below to see Dr. Palm demonstrate how Sculptra in placed along the decollete area for chest rejuvenation:     Other procedures can be combined with Sculptra treatment for total chest rejuvenation.  Laser and light procedures can improve the appearance of the skin by eliminating color changes and further improving skin texture.  IPL can eliminate brown spots and broken blood vessels.  Photodynamic therapy (PDT) can treat photodamage and precancerous spots called "actinic keratoses."  The eCO2 fractionated laser skin resurfacing procedure can smooth and tighten skin.  Ultherapy can be used in the chest area to tighten and lift loose skin.   Interested in more information on chest rejuvenation?  Call us at (858) 792-SKIN (7546) or contact us through the Art of Skin MD Contact page.

Fuller Lips with Filler: Dr. Melanie Palm Perfects the Pout Part II

Juvederm lip san diego   Now that we have explored the use of a cannula to fill the lip in our previous Art of Skin MD post, let's now explore how to create a NATURAL, BEAUTIFULLY-ENHANCED lip.  My philosophy is that we should enhance the inherent beauty of your lips, so that it becomes a harmonious part of the face.  Enhanced lips should never look "done," a frequent concern of patients during initial consultation.   Various fillers are used to augment the lip area.  In the YouTube video below, Restylane is used by myself, Dr. Melanie Palm, to contour the lips.   Other fillers may be used as well.  Juvederm is another popular choice.  A newer hyaluronic acid filler called Belotero can also be used.  Belotero is typically used to fill and smooth fine lines around the mouth but could be used to fill the lips as a more temporary measure.  That way you can "test drive" your lips before you use a longer-term filler like Restylane or Juvederm.  For very select patients that are good candidates, I will use silicone using a microdroplet technique for more permanent lip enhancement.   Pain or discomfort is often a concern of patients undergoing a filler procedure to augment the lips.  I insist that patients are completely comfortable.  I think this is accomplished by several measures.  First, at Art of Skin MD we create an ambiance that invites tranquility and dissolves the anxiety and nervousness that can be associated with undergoing a cosmetic procedure.  The inviting staff, upbeat music, and our smiles will immediately put you at ease.   Beyond the Art of Skin MD culture, we provide other comforts for patients.  The use of both topical numbing and small amounts of numbing injection medicine are used to numb the entire lip area so the lip filling experience is pain-free.  We provide good conversation and distraction tools so that the your visit is a breeze.   Got questions?  Give us a call....it's easy to ask our staff at (858) 792-SKIN (7546).  Or visit our Art of Skin MD contact page.

Perfect Pout by Valentine’s–Lip Filler with Restylane
