Discussing Melasma with Dr. Melanie Palm

One of the things our patients ask about the most, and something we commonly treat at Art of Skin MD, is Melasma.

Melasma is a common, harmless skin condition that causes dark, irregular brown or gray patches to appear, most frequently on the face. Also known as the “mask of pregnancy”, it occurs when pigment-producing cells in the skin overproduce melanin.

Key Triggers:

Melasma flare-ups are primarily driven by three main factors:

  • Sunlight: Ultraviolet (UV) light and visible light stimulate pigment-producing cells.
  • Hormonal Changes: Fluctuations related to pregnancy, birth control pills, or hormone replacement therapy frequently trigger the condition.
  • Heat: Ambient heat and infrared radiation can worsen the appearance of existing patches.

Common Symptoms:

The condition is not physically painful or contagious, but it can cause cosmetic concerns. Symptoms include:

  • Location: Symmetrical patches usually appearing on the cheeks, forehead, bridge of the nose, and upper lip.
  • Appearance: Flat, blotchy patches that can be tan, dark brown, or grayish in tone.

Management and Treatment:

While melasma can be stubborn and may not have a permanent cure, it can be effectively managed through various approaches:

  • Sun Protection: The most vital step in managing melasma is daily application of broad-spectrum sunscreen (SPF 30 or higher) and wearing protective hats.
  • Topical Creams: Dermatologists often prescribe pigment-fading agents or creams containing hydroquinone, tretinoin, corticosteroids, or azelaic acid to regulate melanin production.
  • In-Office Procedures: For stubborn patches, treatments such as chemical peels, microneedling, or laser therapies can help break up the excess pigment. Click here for before and afters!
