Wrinkles (called rhytids) and textural changes, and crepiness around the eyes are common complaints of both male and female patients. These changes in the periorbital region are due to sun exposure, muscle movement, skin thinning, and volume loss. Improvement of crow’s feet, wrinkling, and under eye bags require a proper evaluation of the underlying issue and suitable nonsurgical treatment plan.

Crow’s feet, or crinkling wrinkles around the outside of the eye, are some of the earliest wrinkles to show on the face. Softening of these rhytids and lifting of the eyebrow is possible with neurotoxins such as Botox, Dysport, and Xeomin. Sometimes, a fine filler like Belotero or Restylane Silk can improve etched in lines. Crepiness and textural changes around the eye can be addressed through radiofrequency skin tightening (Protégé Elite), microfocused ultrasound (Ultherapy), and laser resurfacing of the skin (ECO2 laser).

Dark circles around the eyes can be caused by volume loss, thinning tissues and pigmentation. A combination of procedures may be used to improve this area. Injectable fillers (such as Restylane, Restylane Refyne, Belotero, and Volbella) can be used to reduce shadowing or hollowing around the eyes. Pigmentation can be improved through laser treatment (Spectra laser or ECO2 laser resurfacing).

Under eye bags are due to excessive under eye hollowing and/or fat outpouching of the under eye region, referred to as infraorbital fat herniation. Under eye bags, when severe, are best addressed surgically. However, nonsurgical treatments such as advanced injectable filler placement, skin tightening procedures, and laser skin resurfacing can camouflage the under eye bags and improve the youthful appearance of the eye region.
